The Ministry of Education requires a medical PLAN OF CARE for any student that has a life threatening condition.  These include Anaphylaxis, Asthma, Epilepsy, Diabetes, and any other life threatening condition.  This process is new to our Board and forms will only be distributed to those who have identified a medical alert via registration information or the medical letter that will be included in the first day package.  If this applies to your child, please complete these forms promptly and return them as soon as possible.  This will be an ongoing process throughout the month of September as this information is entered in to our data base.  More information will provided soon.
